Stephen - Safeguarding Adults Review

The SAR Subgroup of the Birmingham Safeguarding Adults Board considered this case against the SAR criteria and decided it did not meet the statutory criteria for a SAR, however decided to follow its discretionary power to still carry out a review. An Independent person was commissioned to carry out the review.

The review looks at learning following the death of Stephen who had a history of alcohol abuse and rough sleeping.

Adult 1 - Safeguarding Adults Review

The SAR Subgroup of the Birmingham Safeguarding Adults Board considered the case against the SAR criteria and decided that the referral did not meet the criteria for a SAR, however decided to follow its discretionary powers that a review would take place nonetheless as the Board were keen to identify any learning. The review was led by an independent person.

A decision was made not to publish the full report. Agencies involved have been sighted on the final report, learning and recommendation.

Shannon SAR

This review was carried out by the Sandwell Safeguarding Adults Board. This review explores the circumstances leading to the death of Shannon, from Birmingham, who was aged 24 when she was found dead in a mental health rehabilitation unit in Sandwell. The placement was commissioned by the Birmingham Mental Health Joint Commissioning Team. Section 44 of the Care Act 2014 places a statutory duty on Local Safeguarding Adults Boards to undertake a safeguarding adult review if an adult with care and support needs in its area has died and there is reasonable cause for concern about how agencies worked together to safeguard the adult, and the Board knows or suspects that the death resulted from abuse or neglect.
